ACES Enrollment Important Information

Payment

Enrollment is handled separately for each Chinese class section — the weekday Youth Chinese Language Program and the Weekend Chinese Language Program — meaning you must fill out separate enrollment forms and provide separate checks for EACH PROGRAM. Also, be aware that each program has its own deadlines for early and regular enrollment.

Agreements and Policies

It is important to read through the following policies before you enroll (this is also included on the enrollment document).

Parent’s Agreement

• I understand that I am not to leave my child at the ACES site unless there is a ACES staff member present.
• I understand that my child will not be allowed to leave the program with an unauthorized person.
• Only adults (18 years and older ) can be authorized to pick up the child.
• For the Youth Chinese Language Program, I understand that I will be charged a late fee if I fail to pick up my child by 5:50 PM. Instruction ends at 5:30 PM and the pickup window is 5:30 – 5:50 PM. The Youth Chinese Language Program is a single-skill instructional program providing direct instruction in one skill — the Chinese language — from 3:40 to 5:30 p.m. only, with arrival 3:20–3:40 p.m. and pickup 5:30–5:50 p.m.. ACES is not licensed by the State of Texas and is not a licensed child-care facility, day-care center, or licensed before- or after-school program. I understand that participation carries ordinary physical risks, including slips, trips and falls, minor injuries when moving between classrooms or entering and leaving the building, and exposure to communicable illness. The late fee is $10 for each 10-minute period after 5:50 PM.

  Authorization: In case of an emergency, I understand that every effort will be made to contact me. In the event that I cannot be reached, I hereby give permission to the physician selected by the adult leader in charge to secure proper medical services. I hereby release DG Zenith Academy Inc. d/b/a ACES Learning Center, and its staff and aides from all action that may arise from such medical services.
• I understand and acknowledge that the ACES does not offer any medical insurance to protect against injuries, makes no claim to do so, and has no responsibility for any medical expenses incurred.
• I understand that each participant must assume the risk and any related financial responsibility that could result from participation in any of these activities. I agree to assume such risks and such financial responsibility.
• I further understand that neither the ACES nor its workers can be held responsible in the event of accident .

Registration & Book Fee

A $35 registration fee and a $70 book fee apply. The registration fee is not refundable.

Cancellation of Enrollment and Refund Policy

Cancellations of student enrollment must be requested at least 10 days prior to the start of the classes for which a student is registered to be eligible for a full refund. If requested less than 10 days prior to the start of classes, only 50% refund of tuition will be refunded. No refund will be given after the classes start. The registration fee is not refundable. Refunds will also be issued if there is insufficient enrollment for a course (minimum 5 students)
